Patient Information
We try our best to accommodate everyone. We would like to make your time at our office convenient and enjoyable. With that in mind, please note the following: 1) For the safety and comfort of your child, it is very important for them to be accompanied by a parent or legal guardian. If a grandparent, older sibling, or other relation absolutely must accompany your child to the appointment, please contact our office ahead of time to complete the proper oral and written consent. 2) We require 24 hours notice to cancel an appointment. This allows us the opportunity to offer the appointment to another child awaiting treatment 3) If you are more then 10 minutes late for your appointment, we may have to reschedule you for a later date. It is always best to arrive 10-15 minutes prior to your appointment. As moms, we understand how unpredictable children may be. Sickness may occur and emergencies may come up, but please try to adhere to our office etiquette in consideration of us and the other patients. Thank you.
